WHY YOUR PRIVACY IS SO IMPORTANT TO US
Patient Connect is subject to the Privacy Act 1988 of the Commonwealth of Australia since we are an organisation as defined by the Act and we collect and store personal information.
Patient Connect is a national emergency response service. We provide a service to help and assists our customers, who are aged, disabled, infirm or vulnerable, to continue to live independently in their own homes in the community. In the course of our normal operations, Patient Connect collects and uses personal and sensitive information about our customers to ensure delivery of appropriate, timely and quality services.
This policy statement has been prepared to inform you of your rights under the Privacy Act and to help us comply with our obligations under the National Privacy Principles.
In order to provide our patients with quality health care, we need to hold and use information such as your name and address, health care information that you, your treating doctor or other health practitioner may provide to us. This personal information will only be used for the purposes of meeting your health care needs, unless you consent to its use for other purposes, or the law permits its use for other purposes. Please note that we do not collect personal information on all transactions.
The Privacy Act prohibits any person from accessing, using or disclosing any personal information in the possession of Patient Connect, except if that person is performing their duties of providing emergency response and in accordance with the relevant legislation.
To enable Patient Connect to respond to client emergency calls and requests for assistance, we collect the following types of personal information:

Name and contact number of any carers, family members or responders

Name and contact number of the client’s Doctor and other relevant health care providers

Name and contact number of the agency or individual referring the client for care

The frequency and type of services and care we provide to clients
Personal information will only be collected where it is necessary for the client’s primary purpose of being involved with Patient Connect, and where possible will be collected directly from the client.
Patient Connect makes every effort to keep personal information up to date and accurate. This may mean that at times Patient Connect staff will review the personal information held and request verification to its accuracy.
All personal information that we collect and hold is stored in a secure manner. We do not routinely disclose your personal information to other people or organisations, except to provide it to other health care providers, when necessary to provide you with the level of health service that you require.
ACCESS TO RECORDS
You can request access to the personal information that we hold about you. If you make a request, we will require you to fill out the necessary form and pay the required fee. There is no charge for lodging the request to access your information, however there may be a charge for the time involved and associated costs in recovering and providing the information to you.
